If you wish to use Happy Head as a Health Counselor we may collect additional personal information including, but not limited to: licensing information, credentials and educational history, insurance information, company information, and other professional information.
Whenever you use our website, we may collect non-identifying information from you, such as your IP address, zip code, gender, browsing history, search history, and registration history, interactions with the website, location, referring URL, browser, operating system, data usage, data transferred, and Internet Service Provider. As we value your privacy, only anonymized interactions will be collected through the Platform or Site. We may also collect information including but not limited to postings you make on the public areas of our website, messages you send to us, and correspondence we receive from other members or third parties about your activities or postings.
Push Notifications and Usage Data

Although you are entering into an Agreement with Happy Head to disclose your information to us, we do use third party individuals and organizations to assist us, including contractors, web hosts, and others to allow you to access the Site and Service.
Opt Out of Commercial, Non-Commercial Communications and Do Not Track
If you decide to provide us with your contact information, you agree that we may send you communications via text and emails. However, you may unsubscribe from certain communications by notifying Happy Head that you no longer wish to receive these communications, we will endeavor to promptly remove you from our once we have received that request. We currently do not offer functionality for you to opt out through “do not track” listings. Happy Head may post links to third party websites on our Site or Service, which may include information that we have no control over. When accessing a third party site through our Site or Service, you acknowledge that you are aware that these third party websites are not screened for privacy or security issues by us, and you release us from any liability for the conduct of these third party websites.
We make reasonable attempts to protect your information by using physical and electronic safeguards. For this reason we use SSL certificates to enhance our Site security and encryption to enhance our application security. However, as this is the Internet, we can make no guarantees as to the security or privacy of your information. Your California Privacy Rights
We intend to fully comply with American and international laws respecting children’s privacy including COPPA. Treatment, Payment and Health Care Operations
Happy Head is permitted to use and disclose your PHI for purposes of (a) treatment, (b) payment and (c) health care operations or as otherwise listed. For example:
Treatment. Happy Head may disclose your PHI to providers for health treatment.
Payment. Happy Head may use and disclose your PHI to your health insurer or health plan in connection with the processing and payment of claims and other charges.

As Required by Law. Happy Head may use and disclose your PHI to the extent required by law
Under special circumstances, Happy Head may also disclose your PHI for the following reasons:

Happy Head may disclose your PHI to public health authorities or other governmental authorities for purposes including preventing and controlling disease, reporting child abuse or neglect, reporting domestic violence and reporting to the Food and Drug Administration regarding the quality, safety and effectiveness of a regulated product or activity.

Organ Procurement. Happy Head may, under certain circumstances, use or disclose PHI for the purposes of organ donation and transplantation.
Threat to Health or Safety. Happy Head may, under certain circumstances, use or disclose PHI if necessary to prevent or lessen a serious and imminent threat to the health or safety of a person or the public.
Specialized Government Functions. Happy Head, may in certain situations, use and disclose PHI of persons who are, or were, in the Armed Forces for purposes such as ensuring proper execution of a military mission or determining entitlement to benefits. Happy Head may also disclose PHI to federal officials for intelligence and national security purposes.
Your Rights Under HIPAA
Confidential Communication. You have the right to receive confidential communications of your PHI. You may request that Happy Head communicate with you through alternate means or at an alternate location, and Happy Head will accommodate your reasonable requests. You must submit your request in writing to Happy Head.
Restrictions. You have the right to request restrictions on certain uses and disclosures of PHI for treatment, payment or health care operations. You also have the right to request that Happy Head restrict its disclosures of PHI to only certain individuals involved in your care or the payment of your care. You must submit your request in writing to Happy Head. Happy Head is not required to comply with your request, but may do so at its discretion.
Inspection and Copies. You have the right to inspect and copy your PHI. You must submit your request in writing to Happy Head. Happy Head may impose a reasonable fee for such costs. Happy Head may deny your request to inspect and/or copy your PHI in certain limited circumstances.
Amendment. You have a right to request that Happy Head amend your PHI if you believe it is incorrect or incomplete, and you may request an amendment for as long as the information is maintained by Happy Head. You must submit your request in writing to Happy Head and provide a reason to support the requested amendment. Happy Head may, under certain circumstances, deny your request by sending you a written notice of denial.
Accounting of Disclosures. You have a right to receive an accounting of all disclosures Happy Head has made of your PHI. However, that right does not include disclosures made for treatment, payment or health care operations, disclosures made to you about your treatment, disclosures made pursuant to an authorization, and certain other disclosures. You must submit your request in writing to Happy Head and you must specify the time period involved, which must be within six (6) years of your service date. Happy Head may charge you for such disclosures.
Breach Notification. You have the right to be notified in the event that Happy Head (or a Happy Head Business Associate) discovers a breach of unsecured PHI.
